The product’s core AI wasn’t just inaccurate; it was actively “hallucinating”. It generated absurd, nonsensical text completely unrelated to the medical consultations it was supposed to transcribe. This created professionally dangerous outputs, completely shattered user trust, and caused users to abandon the platform in droves. Furthermore, the application suffered from crippling functional gaps. Clinicians had no way to access centralized patient or document lists, nor could they export or share completed reports. How could the business survive when its primary feature was not only non-functional but actively harmful?
Solution
We moved beyond off-the-shelf prompts to build a robust, scalable custom architecture. We developed a middleware layer that “sanitizes” and “stylizes” inputs with SEO constraints and negative keywords before the AI ever touches them.
AI Stack Overhaul:
We replaced the failing, obsolete model with state-of-the-art solutions in the GPT-4o class models. This single move immediately eradicated the “hallucination” problem and elevated accuracy to a clinical-grade standard.
Real-Time Engineering:
We engineered an innovative real-time transcription mode from the ground up, integrating Azure Speech to leverage the client’s existing resources. We built a novel “placeholder-tag” mechanism —a scalable solution that converts any standard text field into a live, interactive transcription interface.
Core Functionality:
We built the essential data management interfaces clinicians required, delivering clean, responsive Patient Lists and Document Lists with full search and sort capabilities.
Workflow Tools:
We empowered clinicians with critical export tools, including one-click, client-side (cost-saving) exports to PDF and DOCX and a “Send via Email” function.
